Amgen and AstraZeneca presented practice-changing Phase 3 clinical data at the European Society for Medical Oncology (ESMO) 2026 Congress, demonstrating that frontline maintenance therapy combining Imdelltra (tarlatamab-dlle)and Imfinzi (durvalumab) cut the risk of death by an unprecedented 58% (Hazard Ratio [HR] = 0.42) compared to Imfinzi monotherapy in adult patients with extensive-stage small cell lung cancer (ES-SCLC).

The randomized Phase 3 DeLLphi-305 trial evaluated the dual immunotherapy combination as first-line maintenance following standard platinum-etoposide chemotherapy induction. Results showcased a clinically meaningful survival advantage, with the combination extending median overall survival (mOS) to 21.1 months versus 12.7 months in the control arm, with approximately 80% of patients receiving the doublet alive at 12 months compared to 53% on monotherapy. Furthermore, the combination reduced the risk of disease progression or death by 66% (HR = 0.34), showing consistent survival benefits across complex patient cohorts, including those with treated or asymptomatic central nervous system metastases.

Small cell lung cancer represents an exceptionally aggressive neuroendocrine malignancy historically marked by rapid progression, early visceral metastases, and steep post-induction relapse rates. Imdelltra, a first-in-class bispecific T-cell engager (BiTE) targeting delta-like ligand 3 (DLL3) and CD3, works synergistically with AstraZeneca’s anti-PD-L1 checkpoint inhibitor Imfinzi to redirect cytolytic T-cell activity while dismantling immunosuppressive tumor microenvironments. The safety profile remained consistent with previously reported data for each agent, with low-grade cytokine release syndrome (CRS) predictably managed through established step-dosing protocols.

Both companies announced plans to submit these late-stage data to global health authorities to support regulatory filings aimed at moving the combination into first-line maintenance, potentially establishing a new global standard of care for extensive-stage disease.
